Trometamol as a name usually occurred which it is as pharmaceutical intermediate. And its main application is as buffer to adjust pH value in biochemistry and molecular biology. It is also used as pharmaceutical adjuvant. In the following, application of trometamol as pharmaceutical intermediate is described.

Trometamol as an organic amine can react with a poorly soluble drug with an acidic group to produce water-soluble salt.This is conducive to drug ADMET pharmacokinetics. The representative drugs are dexketoprofentrometamol, fosfomycintromethamine, carboprosttromethamine, ketorolac tromethamine etc.

Dexketoprofen trometamol

It is the product with reacting D-ketoprofen with trometamol which shows good water-solubility. It inhibits the biosynthesis of prostaglandins by reversible inhibition of cyclooxygenase and deoxygenase to alleviate pain and exert anti-inflammatory effects. It is as a non-steroidal anti-inflammatory drug to treat short-term trauma and postoperative pain, soreness, severe pain etc.

Fosfomycin tromethamine

It is used for the treatment of non-complex urinary tract infections. The mechanism is that fosfomycin inhibits enzyme activity by binding to uridine diphosphate N-acetylglucuronol-pyruvate transferase and blocks the initial formation of cell wall. It is used for the treatment of urinary tract infections with good safety.

Carboprost tromethamine

It is mainly used for routine treatment of ineffective uterine contraction relaxation, soft birth canal laceration, postpartum hemorrhage caused by incorrect placenta position, and abortion during pregnancy of 13 to 20 weeks. Its function is equivalent to a calcium ion carrier, which can increase the calcium ion transport capacity from the calcium channel into the cell, thereby increasing the intracellular calcium ion concentration and promoting the contraction of the muscle fiber. Meanwhile, it inhibits adenylate cyclase to reduce the phosphorylation of sarcoplasmic reticulum proteins, thereby reducing binding to calcium ions and increasing intracellular calcium ion concentration.

Ketorolac tromethamine

It is used principally as an analgesic, and also used to treat allergic conjucttivitis. Ketorolac is used intramuscularly, intravenously, or orally as the trometamol salt in the short-term management of moderate to severe postoperative pain.

In a word, trometamol as intermediate of four representative drugs is described. The function of trometamol is to make drugs more water-solubility in vivo. So the drug can be adsorbed easily and exert good curative effect.
